FORM: Authorization For Agent To Consent To Medical Treatment For A Minor (August 08, 2001)
Can someone other than a minor's parent or guardian be designated to consent to medical treatment for the minor?
Yes. A child's parents or legal guardian may sign a statement authorizing someone else (friend, relative, camp director, babysitter, etc.) to consent to medical care in the event the child or the child's parents will be away from home, such as during vacations. (Family Code §6910)
